We expect the Caregivers we refer to be: * Reliable – your clients depend on you * Caring & compassionate – your clients need help * Dedicated to making a difference in people’s lives * Competent with smart phones to clock in & out in compliance with Medicaid requirements * Professional and safe in all interactions with clients and their family members
To contract with us for referrals, you will need a copy of the following documents: * Level 2 background screening * Local background check from Police Department or Sheriff’s Office * C.N.A. License or HHA Certificate Or LPN / RN * Driver License & Social Security Card (or Federal Tax ID) * Auto registration & proof of Auto Insurance * Professional Liability Insurance * CPR certification * Physical examination, within the past 6 months, to show free and clear of Communicable Diseases. * First Aid Course (live not online) approved by Agency for Persons with Disabilities (APD) * High School Diploma or GED (for APD cases only) * Voided check or equivalent * CEU certificates
